Enclose this form along with the merchandise in a secure package. Attach below return label to the outside with your Order # and return through M&P. Return Courier Fee will be paid by Customer.

Return Packaging Guidelines

  • All returns MUST include original manufactures box and all its contents in original
  • DO NOT mark or tape original manufactures box in
  • Original manufactures shoebox MUST be shipped back with a protective outside box /
  • DO NOT return shoes in only the original manufactures
  • Damaged packaging (original manufactures box) by customer will result in a $5.00 box
  • It is recommended that you insure the package for delivery back to We cannot be responsible for packages that we do not receive or that arrive damaged, due to shipping.
